What is an mRNA cancer vaccine?

Its not a drug that treats one cancer, its a vaccine designed to prevent cancer itself, and European trials just made it real.

The approach trains the immune system to attack molecules that appear when cells turn cancerous. BioNTech, the company behind the mRNA COVID vaccine, runs individualized neoantigen vaccines that read a patient’s tumor, pick its unique markers, and teach the immune system to hunt anything carrying them. mRNA platforms mean a new cancer vaccine can theoretically be designed in weeks.

The Paradigm Shift: Personalized mRNA Neoantigen Vaccines

Traditional vaccines protect healthy populations against infectious pathogens by introducing harmless versions of viral or bacterial antigens. Cancer vaccines, however, are fundamentally different.  

The European and global trials advancing personalized mRNA therapies—most notably led by Mainz-based BioNTech in collaboration with Genentech/Roche—are therapeutic vaccines designed for secondary prevention. Rather than a universal shot administered to healthy individuals before any illness occurs, this approach targets patients who have already developed cancer.  

The platform relies on individualized neoantigen specific immunotherapy (iNeST):  

1. Biopsy and Sequencing: A patient’s excised tumor tissue and healthy blood cells undergo next-generation sequencing.  

2. Computational Neoantigen Prediction: Machine-learning algorithms identify somatic mutations unique to the cancer cells—markers called neoantigens—that are absent from normal, healthy tissue.  

3. Rapid mRNA Synthesis: An individualized mRNA blueprint encoding up to dozens of these specific neoantigens is manufactured in a matter of weeks and encapsulated in lipid nanoparticles.  

4. Targeted T-Cell Priming: When injected, the mRNA instructs the patient’s dendritic cells to translate the neoantigens and train cytotoxic T cells to hunt down and eliminate any residual, circulating micrometastases.  

The primary goal is preventing recurrence in the adjuvant setting (e.g., following surgical resection and chemotherapy in cancers like melanoma, pancreatic ductal adenocarcinoma, and colorectal cancer).

Because these individualized neoantigen therapies are classified as investigational advanced therapy medicinal products (ATMPs), they are not available “off the shelf” at commercial clinics. Access is strictly managed through authorized academic comprehensive cancer centers participating in clinical trials.
